All players must fill out a RELEASE AND WAIVER OF LIABILITY.  If you are under 18 (10-17), this form must be signed by a parent or guardian.

Those under age 16 (10-15) a EMERGENCY MEDICAL PERMISSION FORM must also be filled out in addition to the waiver.  These are insurance required forms to help protect your safety so please read and fill them out carefully.

You must be at least 10 years old to play.
